Joining Superior’s Preferred and Choice networks is as easy as completing a few simple forms! You may complete an abbreviated form to notify us that you intend on completing the full application at a later date. This short form called Card of Intent allows us to enter your name in The Preferred Plan and/or The Choice Plan printed directories as well as our online network listings.  Or you may choose to complete and submit your full application right away.  You may sign up for both networks using both the Card of Intent and the full application.  If you wish to belong to both The Preferred and The Choice Networks, please make a notation on the form before submitting your information to Superior.
